III. Your Rights Regarding Your Protected Health Information

You have important rights regarding your Protected Health Information. Subject to certain legal exceptions, you have the right to inspect and obtain copies of your medical records, request amendments to information you believe is incomplete or inaccurate, request restrictions on certain uses or disclosures of your information, request confidential communications by alternative means or at alternative locations, receive an accounting of certain disclosures of your Protected Health Information, obtain a paper or electronic copy of this Notice of Privacy Practices, designate a personal representative to exercise your rights when authorized by law, and file a complaint if you believe your privacy rights have been violated.

Although we will carefully consider all reasonable requests for restrictions, we are not required to agree to every requested restriction except where otherwise required by law.

As part of providing telemedicine services, your Protected Health Information may be transmitted electronically through secure communication platforms, electronic medical record systems, patient portals, pharmacies, laboratories, and other authorized healthcare technologies. RHW Precision, LLC utilizes commercially reasonable administrative, technical, and physical safeguards to protect the confidentiality and integrity of your information. However, no method of electronic communication or data storage can be guaranteed to be completely secure, and you acknowledge the inherent risks associated with electronic transmission of health information.

V. Our Legal Responsibilities

RHW Precision, LLC is required by law to maintain the privacy and security of your Protected Health Information, provide you with notice of our legal duties and privacy practices, comply with the terms of this Notice currently in effect, and notify affected individuals following a breach of unsecured Protected Health Information when required by applicable law. We reserve the right to revise this Notice at any time. Any revised Notice will apply to all Protected Health Information maintained by RHW Precision, LLC and will become effective upon posting to our website unless otherwise required by law.

VI. Complaints

If you believe your privacy rights have been violated, you may file a complaint with RHW Precision, LLC without fear of retaliation. Complaints may also be submitted directly to the Secretary of the U.S. Department of Health and Human Services, Office for Civil Rights. Filing a complaint will not affect your access to healthcare services or the quality of care you receive.
